This document contains screenshots of the self-registration process.  See the [[#legend]] at the end for explanations of numbered graphics, fields and other elements that can be customized.

The main index page, containing program titles & general information.  Clicking on a link takes the reader to the self-registration page for that program.  Only enabled programs will be shown.

===== Self-registration page =====

The self-registration page will show only those fields selected on the ReadingRecord setup form.  Fields can vary by program.

===== Registration successful page =====

The "registration successful" page is displayed after a reader has registered for your reading program.

===== Legend =====

  - Main banner.  This graphic can be a library logo, banner with reading program dates, or both.  Recommended size is 698px x 200px (logo) or 698px x 112px (banner).
  - Program title.  Text from tab 2 (Program Info), row 3 of the customization spreadsheet.
  - Program info.  Text from tab 2 (Program Info), row 7 of the customization spreadsheet.
  - Program message.  Text from tab 2 (Program Info), row 6 of the customization spreadsheet.
  - Home library field (optional).  Contents of the drop-down list taken from tab 3 (Branches) of the customization spreadsheet.
  - First name field (required).
  - Last name field (required).
  - Phone number field (required).  The phone number label & field can be configured for 7 or 10 digit phone numbers.  When 10-digit phone numbers are used, the field can be pre-populated with a default area code.
  - Age field (optional).  Contents of the drop-down list taken from tab 6 (Ages) of the customization spreadsheet.
  - City field (optional).  Contents of the drop-down list taken from tab 4 (Cities) of the customization spreadsheet.
  - Zipcode field (optional).  Contents of the drop-down list taken from tab 5 (Zipcodes) of the customization spreadsheet.
  - E-mail address field (optional).
  - Library barcode field (optional).
  - Grade field label.  The label can be customized, defaults to "Last grade completed".
  - Grade field (optional).  Contents of the drop-down list taken from tab 7 (Grades) of the customization spreadsheet.
  - School field label.  The label can be customized, defaults to "Last school attended".
  - School field (optional).  Contents of the drop-down list taken from tab 8 (Schools) of the customization spreadsheet.
  - "Registration successful" message.  The portion in the red rectangle is customizable per program, text taken from tab 2 (Program Info), row 8 of the customization spreadsheet.
