Options for running ReadingRecord should check with their providers.
To get your site setup, do the following:

Download the “2019 setup form” (Excel spreadsheet) at the end of this document.
Fill out the applicable tabs on the setup form, referring back to the documents above as necessary.

You will receive an e-mail when you site is ready for testing.
Test your site to make sure all options & customizations are configured as you'd like. Request changes as necessary.
Added “Program start date” (tab 2-Program Info)
Added “Program end date” (tab 2-Program Info)
Added “Prize / staff instructions -OR- drop-down list items” verbiage (tab 17-Levels)

Note: This form was created with
MS Office 2016 / 365 and contains Excel macros to make the drop-down fields work. It
should work with
MS Office 2013,
